Employee Benefit: Vision
Vision coverage is provided by Vision Service Plan (VSP). The VSP plan covers your periodic examinations to determine any visual problems or other abnormalities. Vision care benefits include examinations and corrective lenses. The employee share of family vision coverage is $2.24 per month
If you use a participating in-network doctor, your vision services are covered at 100% with a $10 co-payment for the examination and a minimum $20 co-payment if materials are required.
If you choose to be treated out-of-network, you can go to a licensed optometrist, ophthalmologist or optician of your choice. VSP will reimburse you up to amount allowed.
Eye exams and lenses are covered once every 12 months, frames once every 24 months. Elective contact lenses are covered either in-network or out-of-network up to $120. Contact lenses are in lieu of lenses for your frames, you may not obtain both in the same 12-month period.
