This application is for persons selling, buying, or refinancing a home in Scott County and the property has a septic system and/or water well that a mortgage company requires inspected.
Print the form, complete all the information, and return it with payment to Scott County Health Department, 600 W. 4th St., Davenport, Iowa 52801.
Upon receipt of the form and payment, an Environmental Health Specialist will contact you to set up an appointment for the inspection.
