Immunization Program
Immunization Clinic:
Walk-in immunizations are available:
Monday through Friday 8:00 a.m. until 4:00 p.m.
Appointments for Immunization Clinics can be made for:
Wednesdays from 7:45 a.m. until 5:00 p.m.
Making
an Appointment:
Call the Health Department at (563) 326-8618 between 8 a.m.- 4:00
p.m., Monday through Friday to schedule an appointment.
Please have the following information available during the call:
- Child's social security number
- Child's previous shot records
Follow-up appointments will be made, as needed, at the end of your visit.
Fees:
There is a $10.00 donation per child per visit for immunizations,
but no one is turned away for inability to pay.
