13 Critical No direct bare hand contact with ready-to-eat foods Employee was observed touching Jello squares with her bare hands.
14 Critical Inadequate protection from cross-contamination In the walk in there was raw chicken above carrots and onions, raw fish above cooked product in reach in across from wok range, raw food above cooked rice in walk in.
22 Critical Inadequate capacity of water system (hot/cold) The hot water handle on the handsink near the wok range isn't operating. Water is being turned off and on at the shut off valve.

29 Non-Critical Floor/wall junctures Thuroughly clean the floors under the dish machine, in the walk ins, under shelving units, under woks and fryers, in the hibachi/sushi area, in the waitress area, in the employee break room and in the buffet area. Clean food debris from the walls in the hibachi area.
31 Critical Inadequate or improper pest control Several roaches were seen in the waitress station near the front door. A roach was also observed crawling on the pillar in the hibachi/sushi area. Droppings were evident on the pillar near the ceiling and also in the cracks of the overhead shelf in the waitress station.
34d Non-Critical Premises not free of unnecessary items Clean the break room used by the employees, also organize the shelving unit across from the wok range separating the tools from food contact items.
